Week 1: 0.6mg (0.2ml) once daily (20 units) Weeks 2: 1.2mg (0.4ml) once daily (40 units) Weeks 3: 1.8mg (0.6ml) once daily (60 units) Week 4: 2.8mg (0.8ml) once daily (80 units) Week 5 onwards: 3mg (1ml) once daily (100 units) (on-going maintenance dose) If patients can't tolerate a higher dose during dose escalation, the increase can be delayed by one more week
Ibuprofen or other NSAIDs (200-400 mg up to three times daily with food) may be used if paracetamol is insufficient, provided there are no contraindications such as renal impairment, peptic ulcer disease, asthma, pregnancy, cardiovascular disease, or concomitant use of anticoagulants
